The map is taken from the Blaeu-Van der Hem Atlas.
Bottom left: Cum Privilegio / ad decennium.
From 1630 onwards the map was included in various atlases published by Willem and Joan Blaeu, including the Atlas Maior.
In the 'Atlas Maior' the chart is included in part IX, leaf 29.
